NHS staff in Staffordshire can join the Health Library. Complete our application form to become a member.
Joining the Library during COVID-19 restrictions
While the library building is closed due COVID-19 restrictions you can still join the library. Download and complete the MSWord version of the membership form listed below. Email it to the Health Library - firstname.lastname@example.org
When we receive your membership form we will get back to you with your temporary membership details. As soon as the library is back up and running fully we will create a membership card for you and contact you about how to collect your card.

NHS users will need to complete an application form in order to join the Library. This is available from the Health Library enquiries counter. You need to bring your NHS ID badge in order for us to verify your employment. You will then be provided with a library card which you must bring with you in order to use our services. Please quote the barcode number on your card in any correspondence with the Library as this helps us to identify your record.
Membership is available to staff at: University Hospitals of North Midlands (UHNM), North Staffordshire Combined Healthcare, CCGs in Staffordshire, NHS GPs and practice staff within Staffordshire and Midlands Partnership Foundation Trust (MPFT).
Students on placement at NHS Trusts in Staffordshire, which we support, are also allowed to join the Library. Your membership is available for the length of your pacement. Please complete an application form and bring it to the enquiries counter.
Keele staff and students automatically join the Library when you start at University. Your Keele card acts as your library card. You must bring your card with you when you visit the Library, especially to take any books out. Please quote the bar code number on your card in any correspondence with the Library as this helps us to identify your record.
The Library may be available to other individuals according to different circumstances. Users may request limited membership by writing to the Health Library Manager. A charge is made for this service.
