Raising concerns about an incident/event in a placement setting
The Faculty of Medicine and Health Sciences is committed to investigating incidents involving unsafe procedures or practice in all placement environments.
As future registrants of health and social care professions, our students have a duty of care to patients, clients, service users and their carers. It is therefore essential that they are empowered to raise concerns where they believe patient or client safety, treatment, dignity or comfort is compromised.
Students also have a duty to themselves and to other students to ensure they are working in a safe environment.
There are three options available for students to inform the University about concerns arising in a placement setting:
- If a student witnesses a safety concern, discrimination or poor professional practice towards themselves or another student, they should use the Report and Support process.
- If a student witnesses a safety concern, discrimination or poor professional practice towards a patient, service user or their carer, they should use the Raising Concerns process.
- If a student witnesses something requiring immediate attention, they should contact their School via Teams, telephone or email. They will then be supported through the Urgent Escalations process.
Keele students can access support from Student Experience and Support Officers, as well as their Academic Mentors, to complete the relevant form. Guidance is also available within the placement modules on the KLE.
