NHS Library Membership Form for NHS Staff (MSWord)
NHS users who wish to join the library can complete the membership application form, which can be downloaded below.
Download the form and either:
• Complete all sections using Microsoft Word, or
• Print the form and complete it by hand.
To return the form:
• Hand it in at the Health Library counter, or
• Email it to health.library@keele.ac.uk.
If you email your form, we’ll contact you once it has been processed and your library card is ready to collect. Please bring employment verification, such as your NHS ID card, when collecting your card.
