Before coming to see us we ask you to complete an online form.  You can do this by clicking on the green button at the bottom of this screen. Once you have done this we will contact you and offer you a time to come and see us. 

Doing this before your first appointment helps us to understand you more and also means you get extra time with a counsellor to talk through what you have written down.  You might even want to tell us things you didn't feel comfortable writing down.

You will need to have your Keele student number, an email address and the name of your GP or family Dr.  It is a requirement of the service that you are registered with a GP.

If you are are struggling with the form you could ask a tutor, member of staff or friend to fill out the form with you.  It's important though that this is done with you and not for you.

The form is confidential, encrypted and can only been seen by CEW staff.

Sometimes the form can be slow to load, so please be patient.
