All students accepting a place on the MPharm course will be required to have a CRB check. Throughout the course you will be involved in placements, meeting members of the public and interacting with patients and other healthcare professionals. The CRB check helps to ensure that we maintain the safety of all those involved in the MPharm programme.

The CRB form will be sent to you shortly before you arrive at Keele to begin your course. You cannot obtain the application form directly from the Criminal Records Bureau so if you lose your form you will have to request a new one from the School of Pharmacy (SoP) Programme Office.

You will need to complete the form and bring it, along with your supporting evidence, to Samantha Walters in the SoP Programme Office by the end of October in the year you start the course.
Remember, the supporting documents you submit with your CRB form must be originals, we cannot accept photocopies.
You should bring one of the following:
- valid passport
- driving licence
- birth certificate
- marriage certificate
- other identification that includes a recent photograph of the student
You should also bring two of the following:
- recent utility bill
- recent credit card statement
- recent bank statement
- recent mortgage statement
- recent insurance statement
Finally, you will need to bring a cheque (made payable to Keele University) or cash to the value of £31 to pay for the CRB check.
Unfortunately we are not able to accept previous CRB disclosures, even if they has been completed recently.

Remember, it is essential that you complete your CRB submission before the schedule of placements begins in November. CRB disclosures take some time to process so you must submit your form and supporting documents by the end of October.
