North Staffordshire Headache Survey Questionnaire
The North Staffordshire Headache Survey questionnaire was developed to measure headache and associated health care use in the general population. We conducted a cross-sectional survey in random samples of adults (aged 18 years and over) registered with five general practices in North Staffordshire in 2000. All responders were re-surveyed after 12 months and a sub-sample was surveyed with a shortened version of the questionnaire every three months.
